If you (and any coauthors) hold the copyright for the previously published work you are reusing, then no, you do not need to submit a permission letter.
If you do not hold the copyright for the previously published work you are reusing, then yes, you need to submit a permission letter from the copyright holder. The copyright holder is usually the publisher, not the journal.
To upload permission letters to ProQuest, select yes when asked if you have copyright permission documents to submit during the submission process.
